Bids exceeding $19,999.99 must be sent as a sealed bid. Indicate on the outside of the envelope that it is a response to an E-Rate Form 470 and that it is not to be opened before the bid evaluation. Multi-year contracts and voluntary extensions are accepted. Please include the voluntary contract renewal deadline on the contract. SPAM and/or robotic responses will not be considered valid bids and will be disqualified. The bidder must agree to participate in the USF Program (AKA "E-Rate") for the corresponding funding year. All bidders must ensure they follow all current E-Rate program guidelines established by the Federal Communications Commission (FCC). Information regarding the eligibility of goods and services, invoicing requirements, documentation requirements, and other program rules is available from the SLD by calling the Schools and Libraries Division (SLD) of the Universal Service Administration Corporation (USAC) at 18882038100 or seeing their website at https://www.usac.org/e-rate/. All bidders must adhere to the Lowest Corresponding Price (LCP) guidelines (as the SLD dictates). All applicable fees, surcharges, taxes, and installation charges must be identified on the bid. E-Rate rules require applicants to evaluate the prices of eligible goods and services to be listed on the 471 application during the competitive bidding process. Fees and surcharges that apply but are not listed on the bid will be the responsibility of the bidding company, should their bid be awarded. Provide detailed item pricing and descriptions; identify and break out associated costs on ineligible items. Note: The vendor is responsible for checking the USF Eligible Services List for updates and corrections before submitting bids/quotations. The bidder must be in good standing with the Federal Communications Commission (FCC) and not have a Red Light status. If the vendor has a Red Light status, the contract will become null and void without penalty to the applicant. The bidder must agree to provide the applicant with the choice of discount methods (SPI or BEAR). The winning vendor will be required to promptly provide the requested information regarding goods and services. All bids must be submitted with the understanding that implementation may be dependent upon the following: (i) Final approval of next year's fiscal budget; (ii) Contract confirmation by next year's board of education; (iii) Award of associated E-Rate funding; (iv) Terms and provisions of the Kansas Cash Basis Law K.S.A 10-1101. The bidder may provide bids for any or all portions of this proposal. The applicant reserves the right to purchase all, none, or a portion of the quoted equipment/services and may choose more than one provider. Bids received after the stated deadline may be deemed non-responsive. Eligible entities: 3.
